I have not taken any illicit drugs and am not under the influence of alcohol prior to commencing my shift today.
I have not taken any prescription medication that can impair my ability to perform my tasks as a ride attendant or operator.
If I take any daily prescription medication for the safety of myself and my co workers I will list them below in my declaration.
In the event that I am unable to communicate for myself, I agree to let medical personal aware of my medication requirements.
My medication is kept in a safe place and can be found in my personal belonging below. I agree that my medication can be sourced if I am unable to communicate for myself.
I understand that it is against my employer and or contractor policy perform my daily work duties if I am under the influence of drugs, alcohol or prescription medication that can impair my ability to perform my role as an attendant or a operator of an amusement device.
I understand that the use of mobile phone and or entertainment devices that can distract my attention of the safety of patrons is strictly prohibited while I am performing my duties as a ride attendant and or operator
I agree to maintain a good level of personal hygiene and professional presentation while performing my duties as a ride attendant and or operator. This includes clean uniforms, neat clean tidy appearance. Facial hair is to be clean shaven and or personal grooming and maintenance of facial hair.
I am aware that poor personal hygiene can affect the wellbeing of myself and my co workers and that body odour can be reduced by regular showers, brushing my teeth and washing my work uniforms.
I am aware that good foot protection is important to be able to perform my duties as a ride attendant and or operator and that comfortable closed in shoes is required with clean dry socks.
I am aware that smoking is NOT permitted while I am working and should be done on my designated breaks and in accordance with the event policy.
I understand that I should not eat meals or snacks while I am performing my work duties and food will be consumed on my designated breaks.
I am aware that it is policy of my employment and or contractors agreement not to have friends or relatives visit or socialize while I am on duty.
I understand that if I have personal commitments that are inhibiting me from being able to perform my duties as a ride attendant or operator that I will make management aware so that I can be replaced by another person that is duly trained in your role.
I am aware that rest between shifts is important and that I have had at adequate rest to perform my duties as a ride attendant or operator.
I am aware that working with children is part of my role and I have taken steps where reasonable practical to obtain police clearance.
I state clearly that I have no pre existing injuries prior to starting work today.
